Great Lakes Health Connect partnering with Vorro to implement a clinical interface
Grand Rapids, MI / Jacksonville, FL – May 20, 2016 – Congress passed the Improving Medicare Post-Acute Care Transformation Act of 2014 (the IMPACT Act) on September 18, 2014. The Act requires the submission of standardized data by Long-Term Care Hospitals, Skilled Nursing Facilities, Home Health Agencies and Inpatient Rehabilitation Facilities. Great Lakes Health Connect (GLHC) is helping to make this a reality in Michigan by partnering with Vorro to implement a clinical interface – BridgeGateHealth™ – to allow for the transfer of clinical data from Skilled Nursing Facilities and Home Health Agencies.
BridgeGateHealth™ extracts clinically relevant patient information from the MDS (Minimum Data Set) and OASIS (Outcome and Assessment Information Set) that nursing homes and home health agencies are required to submit to the Centers for Medicare and Medicaid and converts the information into a standardized Continuity-of-Care Document (CCD).
Great Lakes Health Connect is focused on improving communication among healthcare providers to provide the right patient care and experience at the right time based on the best available information. Their Virtual Integrated Patient Record (VIPR) solution offers healthcare providers real-time access to a comprehensive longitudinal view of a patient’s health record and treatment history. As a condition for access to VIPR, network participants are required to contribute patient data to the database. This reciprocal arrangement delivers the greatest possible value for all. Implementing Vorro’s BridgeGateHealth™solution and providing access to the VIPR solution to the long-term post-acute care market is another step toward meeting their goal of “creating care-connected communities”.
