f you’re evaluating a healthcare interoperability platform, you’re probably not looking for fluffy promises. You want to know what it is, what it replaces, what it connects to, and what it’ll cost you in time and pain. Fair.
I’ve watched great teams ship “working” interfaces that quietly break care coordination for weeks. And I’ve also seen lean interoperability programs hit 99.9%+ interface uptime because they treated interoperability like a product, not a one-time project. That’s the difference we’re going to talk about.

Data normalization, identity matching, deduplication
This is where interoperability becomes trustworthy. You need normalization of demographics, addresses, providers, and clinical content, plus terminology mapping to common vocabularies.
Identity resolution matters more than many teams admit. A Master Patient Index or MPI-like capability with deterministic and probabilistic matching can drive match rates from the low 90s to 97%+ in some environments. That delta is huge when you’re trying to build a longitudinal record.
Workflow orchestration and routing
Routing is not just “send from A to B.” It’s conditional logic, enrichment, retries, dead-letter queues, and idempotency so you don’t create duplicates when a downstream system times out.
ADT triggers are a classic example. An admit event should kick off care management enrollment, notify a payer, and update a patient-facing timeline. But only once. And only for the right patient. Sounds simple, right?
Monitoring, alerting, and audit trails
If observability is weak, you’ll pay for it later. You want dashboards for throughput, latency, and error rates, plus searchable message traces and audit logs.
And don’t accept “we have monitoring” as an answer. Ask: can I see an error queue, replay safely, and prove what data was sent to which endpoint at what time? If not, you’ll be triaging by screenshots. Nobody wants that.

And yes, bulk data matters for analytics, population health, and payer reporting. Ask how the platform handles large exports, throttling, pagination, and retries without taking down your API tier.
Terminologies
Semantic interoperability doesn’t happen by magic. You need terminology alignment across LOINC for labs, SNOMED CT for clinical concepts, and ICD-10 for diagnoses and billing-driven workflows.
So, does the platform include a terminology service, or do you bring your own? Either can work. But you must plan for ongoing mapping maintenance, because codes change, labs add new tests, and local codes never fully disappear.

Start small, but don’t start sloppy. A phased roadmap works best.
- Pilot: 1 to 3 high-value interfaces, clear success metrics, and a real on-call process.
- Foundation: establish naming conventions, mapping standards, identity strategy, and monitoring dashboards.
- Scale: onboard additional facilities and partners using repeatable templates and automated testing.
- Optimize: reduce mapping variance, improve match rates, expand eventing, and formalize governance.
And don’t ignore training. The best platform fails if only one engineer knows how to troubleshoot it.

Interoperability increases your blast radius. That’s not fearmongering. It’s reality. So security and privacy must be baked into design, not bolted on after go-live.

And ask for references that match your world. A digital health startup reference won’t help a 12-hospital system with 40+ downstream apps.

Total cost is rarely just “license.” Watch these drivers:
- Interfaces and endpoints: Per-interface pricing can punish growth and M&A.
- Transactions: Message volume pricing can spike with ADT eventing and bulk exports.
- Terminology: Some vendors charge separately for terminology services or value set management.
- Environments: Dev, test, staging, and prod add up, especially in hosted models.
- Support tiers: 24/7 support, named TAMs, and incident response SLAs cost real money.
So, model costs with your next 24 months of growth, not today’s footprint. Otherwise you’ll “win” the purchase and lose the program.
Proof-of-concept success criteria
A proof-of-concept should be short and strict. Two to four weeks is plenty if the vendor is real.
- Connectivity: Stand up one HL7 v2 feed and one FHIR workflow with auth.
- Reliability: Demonstrate retries, replay, and idempotency without duplicate downstream records.
- Monitoring: Show dashboards, alerting, and audit evidence for a test incident.
- Data quality: Prove normalization and terminology mapping on a sample set.
If a vendor can’t do that in a POC, scaling will be painful. That’s just how it goes.

Data quality is not a one-time cleanup. It’s a living metric. Treat it that way.

- Reduced duplicate tests: Track repeat labs within 24 to 48 hours when prior results exist.

- Call volume: “Missing results” and “can’t find patient” calls to help desk or HIM.
- Public health timeliness: Time from result finalization to report submission.
If you can’t tie interoperability to workflow impact, you’ll struggle to defend budget when priorities shift.
Common Pitfalls and How to Avoid Them
Most interoperability failures aren’t technical mysteries. They’re predictable patterns. The good news? You can avoid them with a little discipline.
Over-custom mapping, brittle point-to-point
Point-to-point integrations feel fast. Then the third system shows up. Then the tenth. Suddenly every change request is a mini project.
Standardize mappings, reuse canonical models where you can, and keep transformations version-controlled. Also, invest in automated regression testing for interfaces. It sounds “extra” until the first production outage.
Identity and consent gaps
If you don’t solve identity early, you’ll ship features that look fine but harm trust. Clinicians will see duplicate patients. Care managers will call the wrong number. It gets ugly.
Consent is similar. If you can’t express consent rules in a consistent way, you’ll either over-share or block legitimate exchange. Neither is acceptable.

Does FHIR replace HL7 v2?

So focus on the basics that matter: connectivity breadth, identity resolution, terminology and normalization, orchestration with retries and idempotency, and serious observability with audit trails. Pair that with a pragmatic roadmap, a clear build vs buy stance, and KPIs that prove clinical and operational impact.
Do that, and interoperability stops being a constant fire drill. It becomes infrastructure you can trust. And honestly, that’s the goal.
