HL7 BTX Segment: Blood Product Transfusion/Disposition

Overview

The BTX (Blood Product Transfusion/Disposition) segment communicates the final transfusion or disposition outcome for a blood product previously ordered with a BPO and dispensed with a BPX. BTX is delivered inside a blood-product transfusion/disposition response (BTS). It carries the donation ID, component coding, blood group, commercial-product details, quantity and amount, transfusion/disposition status, message status, the date/time of status, the administering and verifying clinicians, transfusion start/end timestamps, any adverse-reaction codes, and the reason a transfusion was interrupted.

When to use BTX

Use BTX whenever the blood bank or clinical unit needs to communicate the disposition of a unit that was previously dispensed: transfused, partially transfused, returned, wasted, or quarantined following an adverse reaction. Each BTX repetition reports one disposed unit and is paired to the matching BPO/BPX repetitions inside the bb-products group.

The donation ID (BPX-5 / BTX-2) is the durable join key across the dispense (BRP) and disposition (BTS) responses, while ORC and BPO carry the same order identifiers throughout the lifecycle.

FHIR mapping

No segment-level ConceptMap is published in the v2-to-FHIR IG for BTX. The information conveyed by BTX maps conceptually to FHIR MedicationAdministration (referencing a BiologicallyDerivedProduct), with adverse reaction data (BTX-18) flowing to AdverseEvent and the administering and verifying clinicians populating MedicationAdministration.performer.

Implementation notes

  • BTX-11 (HL70513) is the clinical outcome — Transfused (TX), Partial Transfusion (PT), Returned (RA), Wasted (WA), etc. BTX-12 (HL70512) is the high-level message status — receiving systems should not rely on BTX-12 alone to drive clinical logic.
  • BTX-18 is the only repeating field — use the ~ separator to convey multiple adverse reactions observed during a single transfusion.
  • Populate BTX-16 and BTX-17 whenever the unit was actually administered; downstream analytics rely on these timestamps to compute transfusion duration.
  • BTX-19 should be populated only when BTX-11 indicates an interrupted or partial transfusion, and should reference the same patient-facing adverse-reaction context as BTX-18 when applicable.
